Please complete and submit this form to receive a unique Returned Materials Authorization (RMA) number for your order. If your order involves a potential warranty claim, please first contact Sierra Instrument's excellent Technical Support Department for possible resolution. If your instrument is returned as a warranty claim, and no Sierra-fault is found during the evaluation process, Sierra's Warranty Policy states that a $100 evaluation fee may apply. If your order does NOT involve a warranty claim, Sierra will provide a quote within 72 hours of receiving your instrument(s). If the quoted charges are not approved, a $100 evaluation fee may apply.

Note: Equipment left at Sierra Instruments for over a calender year will be subject to disposal.
